Hospice services are designed to meet the individual needs of each patient and family. An individual plan of care will be developed by the hospice team along with the patient and family and may include the following:

  1. Hospice medical director visits along with oversight of hospice interventions.
  2. Home visits by registered nurses.
  3. Certified nursing assistants to help with patients' personal hygiene.
  4. Social worker visits.
  5. Chaplain visits.
  6. Durable medical equipment (ie hospital beds, wheelchairs, oxygen etc.).
  7. Medications related to symptom management and pain control.
  8. Assistance with medical supplies (ie diapers, chubs).
  9. Continuous home care during a short-term crisis.
  10. Respite care for up to 5 days in a licensed long-term care facility.
  11. General inpatient care.
  12. Bereavement counseling services.
  13. Volunteer services.
