Where can hospice be provided?

When a patient is referred to a hospice program, their condition is evaluated by a registered nurse, who in consultation with the Attending Physician will prepare an initial plan of care that will include the level of care needed at that time. While many patients receive hospice care in their home, Portsbridge also provides care to patients residing in Long Term Care Facilities, Personal Care Homes and Assisted Living Facilities.


Routine Home Care Program

If the patient has mild symptoms and requires no special care, they can be treated at home.  This Routine Home Care includes but is not limited to the following:

  • RN Case Manager visits
  • Medical Social Worker visits
  • Home Health Aide services
  • Chaplain services
  • Hospice Medical Director
  • Medications
  • Equipment

All of the above are provided as needed to manage the terminal illness and help the patient maintain comfort. How often each member of the care team visits is determined by the needs of the patient. All of the hospice staff works together with the patient's Attending Physician to create a plan of care, which addresses the needs of the patient. In addition to all of the above services, you also have 24-hour access to hospice registered nurses who are "on-call" to answer questions and help you deal with situations that may arise after hours.


Inpatient Hospice

When the patient's symptoms progress and needs are more intense, Portsbridge is committed to taking an aggressive approach to control pain and insure patient comfort. This approach may include moving the patient from their home to our inpatient hospice facility for a short time to provide the aggressive nursing care required to meet the patient's needs. At the Inpatient Unit you can expect:

  • Aggressive individualized attention to assess patient needs and to take immediate action to relieve pain and insure patient comfort.
  • Very close coordination between the hospice staff and the patient's Attending Physician, providing a team to attend to all patient needs.
  • Medication dosages and methods of delivery to attain optimum effectiveness of the medications.
